This Privacy Policy explains what information
we collect from you, why we ask for the information, how we will use
the information, how we will not use the information, and how we will
safeguard the information.
You do not have to provide Personally Identifying Information to visit
our site.
We will collect Personally Identifying Information only when you choose
to provide it, as necessary to provide a MEDMORE Health Benefit. If
you choose to provide Personally Identifying Information, we will use
any it only for the purposes that we state to you when requesting this
information. We will share Personally Identifying Information with partners
only as necessary to administer your MEDMORE Health Benefit. It will
not be shared with anyone else without your prior permission.

Personally Identifiable Information
This Privacy Policy applies to what we call "Personally Identifying
Information" (PII) which is any information that could identify
you or could be used to contact or locate you. Examples include (but
are not confined to) your name or your address.
To become a MEDMORE member or to add a member to your account, you are
REQUIRED to provide PII. This information may be used to enroll you
in our services; to calculate the discounts for which your are eligible;
to charge your for your MEDMORE membership; and to communicate with
you in writing about changes to your MEDMORE membership. This information
includes name, date of birth, and gender; physical and email address,
etc.
In the course of providing your MEDMORE health benefit, MEDMORE may
obtain Protected Health Information (PHI) as defined under the U.S.
Health Insurance Portability and Accountability Act (HIPAA). MEDMORE
may choose to share PHI directly with members over the age of 14, and
with primary members when dependent members are under the age of 14.
If you identify yourself by providing PII or PHI, you should know that
the material you submit may be seen by various people. We may enter
the information you send into our electronic databases, to share with
our vendors or regulatory bodies. You may be contacted by us or any
of these agencies.
MEDMORE also utilizes ‘cookies’, a piece of data stored
on your computer's hard drive when you visit many websites. MEDMORE
uses cookies to track the stage of completion of one of our surveys
or questionnaires if you do not complete it during a particular Internet
session. It allows us to return you to the correct place in the partially
completed questionnaire. MEDMORE does not use cookies to analyze the
movements of the users on our site, and does not use cookies to gather
demographic or other data on our users.
We make extensive efforts to protect your PII but cannot absolutely
guarantee such protection.
Accordingly, you should carefully weigh the advantages and disadvantages
of providing ANY personal information.
When MedMore Will Contact You
We may contact you for reasons pertaining to your membership with us.
This includes sending you membership cards, billing statements, or statements
regarding medical services you received under your membership. You cannot
unsubscribe from membership-related communications, but you can inform
us of your contact preference (email address or physical address). If
you contact us with a question, comment or request for information,
we will make every effort to give you a timely response. We may respond
directly to inquiries and requests regardless of any request by you
to opt-out of marketing or informational mailings. For example, if you
provide your email address and choose to complete a questionnaire or
survey, we may respond to your inquiry or send you nformation you requested.
We may also choose to send you marketing or informational messages from
time-to-time. These communications may be by email or postal mail and
you can ask to ‘unsubscribe’ from these at any time.
When you cancel your membership, you can request that MEDMORE not contact
you under any circumstances except to respond to a later communication
initiated by you.
HIPAA Compliance and Compliance with
State Privacy Laws
The U.S. Health Insurance Portability and Accountability Act ("HIPAA")
controls the collection, use, and distribution of individually identifiable
health information (PHI) by certain health care providers, called "covered
entities" under the statute. Because we may from time-to-time receive
health-specific information about you, MEDMORE is a covered entity as
that term is defined under HIPAA.
Information Collected and Stored Anonymously
This Privacy Policy does NOT apply to information that is collected
anonymously (i.e., that does not identify you) or information about
groups (rather than an individual), which we sometimes use to show group
patterns, trends or geographic groupings.
If you do nothing during your visit but browse through the Web site,
read pages, or download information, we will gather and store certain
non-personal information about your visit automatically:
a Your Internet domain (for example, “xcompany.com”) and
IP address (an IP address is a
number that is automatically assigned to your computer whenever you
are surfing the Web)
from which you access our Web site;
b The type of browser and operating system used to access our site;
c The date and time you access our site;
d The pages you visit; and
e If you linked to our Web site from another Web site, the address of
that Web site.
We use this information to help us make our site more useful to visitors
– to learn about the number of visitors to our site and the types
of technology our visitors use. We do not track or record information
about individuals and their visits.
Technology Available for Privacy Protection
All information that you transmit to our server computer (other than
email information, which should NOT be considered a secure communication
channel) uses SSL (Secure Sockets Layer) technology, an Internet standard
that works by using a private key to encrypt data that is transferred
over the SSL connection. SSL is commonly used for transmitting credit
card numbers safely over the Internet.
Normal email transmissions are not secure. This applies to email between
you and us, or other email that you may send or receive. Although unlikely,
any normal email may be intercepted by unauthorized persons. Accordingly,
you should always be careful when deciding what information to send
in an email. For many of our services and for our mutual ease of communication,
we ask you to provide an email address for contact purposes. Your email
address is not shared with other parties without your specific permission
and no unsolicited emails will be sent.
We protect against hackers entering our server environment by using
a firewall. Data contained on our servers is visible only to proprietary
middleware applications. Sign-on passwords are required by internal
staff and MEDMORE personnel and are changed on a periodic basis. All
internal activity is logged. Our servers are maintained in a secure,
locked, environmentally controlled environment. We regularly backup
our databases and these backups are similarly stored in a secure environment.
We also protect PII (as well as other information) off-line. Any information
we receive, whether defined as PII or not, is restricted in our offices.
Only employees who need the information to perform a specific task (for
example, our enrolling agents) are granted access to your PII. Our employees
and consultants are kept up-to-date on our Privacy Policy. Any time
our Privacy Policy is revised, all employees and consultants are so
informed and reminded of their confidentiality obligations.
Note that there may be periods when this website is temporarily not
SSL-enabled; during such periods, a notice to that effect will be prominently
displayed on appropriate pages of this website. MEDMORE will take all
reasonable steps to protect your privacy but cannot absolutely guarantee
freedom from the risk that your personal data could fall into the hands
of unauthorized persons.
Advertisements
We will not give, lend, sell or otherwise provide your PII to any advertiser.
The only products or services we sell on our Web site are provided by
us or our partners. We do not accept advertisements.
Therefore, no intermediaries such as advertisers will be provided with
your PII.
Web Links
We do our best to provide you with helpful, trustworthy resources, and
we cannot endorse, approve or guarantee information, products, services,
or recommendations provided at a third party Web site.
Because we may not always know when information on a linked site changes,
we are not responsible for the content or accuracy of any third party
Web site. We will not be responsible for any loss or damage of any sort
resulting from the use of a link on its Web site nor will we be liable
for any failure of products or services advertised or provided on these
linked sites.
We offer links to you on an “as is” basis. When you visit
a third party Web site by using a link on our site, you will no longer
be protected by our Privacy Policy or security practices. The data collection,
use and security practices of the linked site may differ from the practices
of our site. You should familiarize yourself with the Privacy Policy
and security practices of the linked Web site. Those are the policy
and practices that will apply to your use of the linked Web site, NOT
our policy and practices.
Use of Intermediaries for Information
Transmission or Analysis
We may use the services of the U.S. Postal Service or other outside
organizations to provide you with information that you have requested.
The U.S. Postal Service has strict policies for proper handling of mail.
We are careful that any other outside organization used to provide information
to you is obligated to use your PII only for the purpose of facilitating
transmission of this information to you. We may send our databases or
portions of our databases to third parties to process and analyze our
data on our behalf for elimination of duplicate records, or other data
cleaning services, or to assist with delivering messages to you. All
such third-party processors are contractually bound to maintain the
confidentiality of your PII.
Data Protection Policy for Non-U.S.
Users
PII collected from users from the European Union will be transferred
to MEDMORE in the U.S. only with your proper permission, and in compliance
with the U.S. Department of Commerce Safe Harbor Certification standards.
MEDMORE’s data security practices are believed to comply with
all European laws on data protection. As far as we are aware, MEDMORE’s
Privacy Policy meets all legal requirements of countries outside the
U.S.
Changes to the Policy
We regularly review our Privacy Policy to ensure maintenance of privacy
and conformity with industry standards. We will post the date of the
most recent version of our Privacy Policy on this web page.
Please understand that your use of our website means that you accept
our Privacy Policy.
Sale or Transfer of Corporate Assets
MEDMORE expressly reserves the right to transfer its database in part
or as a whole to another legal entity as part of a sale or merger of
all or part of MEDMORE with a separate legal entity. Any entity acquiring
any data assets of MEDMORE will be required to provide an express, written
commitment to use the data only for the purposes for which it was provided
to MEDMORE and in accordance with the MEDMORE Privacy Policy. Data assets
will be transferred only after MEDMORE is satisfied that the acquiring
entity can and will provide at least as much protection for PII as MEDMORE
does.
