$30.00* per month will include you, your spouse and all of your children. For only $1.00 per day you can have some peace of mind. *First Month Only Enrollment Fee of $35.00.

DENTAL:

You can choose any Dentist from our Provider list at pre-contracted fees.

Monthly rate:

$59.95* per month will include you, your spouse and all of your children. For only $2.00 per day you can have peace of mind.

We also have even lower rates for individual, couples, and groups.
